There is a straightforward reason Chiang Mai has more good Italian food than a Northern Thai city of its size should: it has been one of the world's largest concentrations of long-stay remote workers for over a decade, and that population eats pizza. What began as expatriate comfort food has had long enough to professionalise — wood-fired ovens, imported flour, in-house pasta, gelato made on site.
The tell is in the data rather than the menus. This is the only category on the site where TripAdvisor covers noticeably more of the list than Google does, and that is a signature: Italian restaurants in Chiang Mai are chosen by visitors and long-stayers who review in English on the platform built for travellers, rather than by Thai diners searching in Thai. It makes the TripAdvisor ranks on this page unusually meaningful and the Google gaps unusually uninformative.
Practically the list divides into the Nimman cluster — smaller rooms, nomad-facing, strong on pizza and coffee — and the riverside and old-town restaurants that operate as proper evening destinations with wine lists. Both are represented below.

Trust the TripAdvisor column here more than usual. It is the platform this category's actual customers use, and it covers more of this list than Google does.
Wood-fired is worth asking about. It is the clearest dividing line between the serious pizzerias and the rest, and the ones that have an oven tend to say so.
Book the riverside rooms. The evening-destination end of this category fills up, particularly in high season, in a way the Nimman pizzerias do not.
